I collaborated with a friend, who has a background as a carer, to form a small team of specialists with medical, web development and creative experience, with the ambition to build Total Care Manager, a tool that meets the needs of case managers and care teams, to ultimately improve the quality of home-based complex care.

The structure of Total Care Manager encourages teams to embrace this complexity; a comprehensive Client Profile enables teams to track a care client’s short-term care needs and unique traits, while keeping in sight their longer-term health goals.

The Client Landing enables care teams to keep on top of their day, delivering scheduled care tasks at the right time, and documenting their responses to unanticipated situations that may arise.
